Membership Terms & Policies
Commitment & Cancellation
• 6-month minimum commitment required for all tiers
• After 6 months: Cancel with 30-day written notice
• Early cancellation fee: $75 (if canceling before 6-month commitment is fulfilled)
• Cancellation takes effect at end of billing cycle after notice period
Rollover Policy
• Unused units/credits roll over for up to 3 months
• Maximum accumulation: 3 months of benefits
• Rollover benefits forfeit upon cancellation
Pause Option
• Pause membership once per 12-month period for up to 2 months
• Valid reasons: Medical necessity, extended travel
• Must provide 14-day advance notice
• Paused months do not count toward 6-month commitment
Transfer Benefits
• 1 transfer per 6-month period to immediate family member or friend
• Transfer must be scheduled in advance
• Recipient must complete consultation and consent forms
• Non-transferable to other businesses or for resale
Payment & Billing
• Automatic monthly billing on enrollment date
• Accepted methods: Credit/debit card, HSA/FSA cards
• Failed payment results in suspended benefits until resolved
• 2 consecutive failed payments may result in membership termination
